You can find the process for making the disc adapter in the link. If I make the combo adapter again, I'll get a .75 drill bit and a 5mm tap from my local hardware store. The 6mm bolt holes leave a very thin margin for error.

Here's the competed part. The freewheel adapter can be bored out to 3/4" with a step drill available at HF, as part of a 3 pc. set that costs less than 10 bucks. Then you just slide it on the axle against the key, mark the keyway slot and cut it with a file or jigsaw.
The freewheel adapters have gone up to I think 13 bucks at bikepartsusa.com. They were 10 bucks each when I bought them. They may be available at Niagara Bike parts also. I suggest using the keyway, as it will help prevent the weld from breaking loose under the pressure of braking.
